Our nomination process is a 3 step process that begins right here. Fill out this form and provide a brief overview of what you are needing assistance for, whether it is for yourself or someone else. Once we receive the request, our board will review the information given and assess if the need is in our realm.
If we are able to provide assistance, we will reach out to set up a call or in person interview to learn more about you (or the person you are nominating) and how we can help directly.
Once we have learned more, the board will review the application and make a final determination, then provide assistance financially or through accessible resources based on need.
All information written and spoken is confidential to only be shared with board members.
